We will also collect some sensitive information such as the reason for appointment, dental insurance information, medical and dental history, experiences you have had in other dental care, expectations of the services to be received by SMILE, treatment, dental health projection, and relevant medical and dental information. When the information is provided directly by you to SMILE, we presume that it is accurate, complete, relevant, correct, and will be kept updated.
Purpose for the use and disclosure of your personal and health information:
The collection, use, disclosure, or storage of your information will be for the following purposes: identification and contact with the patient; creation and updating of the clinical record; preparation, diagnosis, and monitoring of dental treatments; request and storage of studies, clinical analysis of your oral health; billing for services provided; management and collection of medical and dental payment from insurance; manage, schedule remind, and confirm appointments; keep you informed of products, services, promotions, advertising of our services or topics that may be of your interest; satisfaction surveys focused on improving the patient experience and; Internal statistical studies.
Limitation of Use, Disclosure, and Transfer of Personal Information:
Smile commits to maintain the confidentiality of your information and to use it only for the purposes described in this Notice of Privacy Practices. We do not share your information with third parties, except in cases where it is necessary for the provision of our services such as: with dental laboratories, required health studies, subcontracted health personnel to attend you, professional advisors, clinics, hospitals, other providers of dental services, medical insurers, for collection of payment from medical insurance, as well as for the treatment and storage of your information on national or international servers. In all these cases, it’ll be ensured that the third parties with whom your information is shared comply with the appropriate security and privacy measures to protect your information.
Your information will at no time be sold to any company that seeks advertising or profit purposes.
ARCO Rights (Access, Rectification, Cancellation, and Opposition):
For the proper use of your information, we will take reasonable steps to ensure that your information is accurate and up-to-date.
You or your legal representative have the right to access your information, as well as to request that we: amend your information if it is inaccurate or incomplete, cancel your information when you consider that it is not required for any of the purposes indicated in this Notice of Privacy Practices, or oppose the use or disclosure for specific purposes, as long as this is not contrary to what the competent authorities forces us to.
Your request must be in writing and should explain why you are requesting it, for which you must send us an email or submit a written request in our offices to addresses that we have indicated in the first paragraph of this document or through our website to initiate the assignment using your electronic signature through your username and password. To request documentation related to your clinical record can only be done personally and in writing at our office.
The writing indicated in the previous paragraph must contain: Full name and address of the patient, clearly describe the ARCO right you wish to exercise, and accompany it with a valid official document with a photo and signature of the patient that allows us to prove your identity. If you wish to amend your information, you must indicate the information you wish to amend and provide the documentation that supports your request.
Within a maximum period of 30 business days, SMILE will notify you of the response to your request by the same instrument you used to submit it. If we require more information from you to resolve your request, we will contact you via email requesting the required information. If within 10 business days, we do not receive the requested information from you we will consider your request as not submitted. In case of receiving your information within the indicated period, SMILE will resolve your request within 10 working days from receiving the additional information requested.
We will use our professional judgment and our experience with common practices to make reasonable inferences of your best interest. We may deny your request under certain circumstances.

To guarantee the security and reliability of your information, in SMILE we maintain administrative security measures to safeguard and protect the confidentiality of your information.
